Q1What medical equipments can I borrow from the Division of Health Service?

           available for borrowing.

Q2Who can borrow medical equipment from Division of Health Service?

           Only faculty and staff of the school, and you need to bring your student ID card or staff ID card for verification when borrowing.

Q3How to borrow medical equipment?

            Please fill out the medical equipment borrowing application form at the Division of Health Service during working hours and

           Your ID card will be held as identification during the borrowing process and will be returned once the equipment is returned.

Q4: What are the new rules for borrowing first aid kits and first aid packs from July 1, 2024?

          Only those who have attended first aid kit demonstration or first aid education courses for more than 4 hours, and have received

         a certificate within the time limit can borrow the first aid kit and first aid waist pack, and the certificate must be presented for

         verification when borrowing.

Q5: I don't have a certificate related to the first aid kit demonstration or first aid education courses for more than 4 hours, where can I take the class?

         1.On-campus first aid education courses: Classes are held irregularly every year, please check the emergency first aid education

           on the website of Division of Health Service for course information.

        2.First aid kit demonstrationPlease call 03-5715131 ext. 74401 to confirm the teaching time before 1 month, and then go to

           the website of Division of Health Service Medical Equipment Borrowing Health Advisoryto book an appointment.

        3.Off-campus first aid education courses: Enroll in other off-campus first aid education courses for more than 4 hours at your

           own expense and obtain a license.

Q6Is there any restriction on the use of first aid kits and first aid packs?

            First aid kits and first aid waist packs can only be borrowed for on-campus activities and clubs, and each activity or club is based

           on the principle of borrowing one, and no personal borrowing is provided.

Q7Do I need to make an appointment to borrow a first aid kit and first aid waist pack?

             It is recommended to make an appointment at the office of Division of Health Service from 30 days before the start of

            the winter and summer vacations, borrow and receive it 2 days before the event, as to avoid temporary borrowing

            without equipment available.

Q8Can I renew my loan if I can't return it on time due to special reasons?

            Before the return date, you can go to the office of Division of Health Service to fill in the application form again or send a letter to

           the public mailbox of the Division of Health Service. After that, we will confirm it before completing the renewal. The renewal will

           be based on the principle of 1 time.

Q9Notes to be taken by borrowers when borrowing and returning?

            If the equipment is damaged or lost, it must be repaired or returned within 2 weeks.
